Employers with vaccine mandates in place report being inundated with religious accommodation requests that would potentially excuse an employee from getting a Covid-19 vaccine. The form also provides a well-documented data sheet that the employer can file to remain compliant and covered – should something come up.
Specifically, the form directs employers to ask employees about how the proposed policy conflicts with their sincerely held religious beliefs or religious practice.
