EEOC Template for Employers to Evaluate Religious Accommodation Requests for COVID-19 Vaccine Exemptions

Employers with vaccine mandates in place report being inundated with religious accommodation requests that would potentially excuse an employee from getting a Covid-19 vaccine. Employers may find it difficult to judge the validity of a religious accommodation petition. For that, the Equal Employment Opportunity Commission (EEOC) released a template that employers can use to evaluate each request on a case-by-case scenario. The form also provides a well-documented data sheet that the employer can file to remain compliant and covered – should something come up.

Specifically, the form directs employers to ask employees about how the proposed policy conflicts with their sincerely held religious beliefs or religious practice.
